80 year old patient with obstruction of the left hepatic duct. Imaging with EUS, MRI and CT scan didn't show a mass. However, due to the high suspicion of an on going bile duct malignancy we performed cholangioscopy with intraductal biopsies. The final histology showed cholangiocarcinoma. The patient underwent a left hemihepatectomy.
